I certify that my child has permission to participate in activities at Goatfarm E-Sports (GOATFARM). I will allow GOATFARM to reach out to the child’s emergency contact in the event I cannot be reached. I agree to read and discuss guidelines issued by GOATFARM with my child. I hereby grant the GOATFARM ESPORTS, its legal representatives, agents, successors, or assigns, the absolute right and permission to use any photographs, movie films, digital images, videotapes, and/or sound records, or any part thereof, of My Child or in which My Child may be included, in whole or in part, for any lawful purpose, including but not limited to any GOATFARM ESPORTS publication or on GOATFARM ESPORTS websites, whether or not in conjunction with My Child’s name, without pay or any other consideration.
I hereby release, discharge, and hold harmless GOATFARM ESPORTS and its employees from any and all claims, causes of action, and demands of whatsoever nature, including but not limited to any claims of libel, or invasion of privacy, I or My Child have or may have by reason of this authorization and release or use of My Child’s photographs, movie films, digital images, videotapes, and/or sound recordings, or any part thereof depicting My Child.
I hereby waive any right that I may have to inspect and/or approve the finished product wherein My Child’s likeness appears or the use to which it may be applied.
I hereby warrant that I have read the above authorization and release, prior to its execution, and that I am fully familiar with the contents thereof. I represent that I am at least 18 years of age and am fully competent to sign this release.
In consideration for the services received and the risk assumed by me on my own behalf and that of my child, GOATFARM’ owners, directors, employees, contractors, instructors, attendants, and assigns are fully and absolutely released and discharged from all claims of injury, disability, disease, damage, loss, and expense arising out of, or connected in any manner with any accident or injury sustained by my said child and his/her heirs, executors, assigns, and representatives, while said child is engaged in any activity directed, sponsored, conducted, or maintained at or by GOATFARM’ owners, directors, employees, instructors, contractors, attendants, and assigns. The release is executed of my own free will as consideration for access to the services, facilities, and equipment for GOATFARM.
